Observation

2a) items with a diameter less than 1 3/4 inches which were accessible to children on low shelves. plastic "baby proofing" plugs were placed in outlets that can be removed. 2b) blinds and shades with pull cords were seen in the licensed space. 2c) corded beads were seen hanging from the window 2d) plastic bags were seen stored in the hallway diapering area and in the bathroom accessible to children. 3a) sheets of wood were seen in the backyard rotted, with splintered edges with a shelf placed on top, placed on top of wooden boards creating a raised platform a few inches from the ground. a plastic riding toy was seen to have a broken off handle, creating a sharp edge. 3f) shelving was seen not secured to the wall posing a tipping hazard. Items were stacked and placed on shelves over hanging the edge which could potentiallfall off 3g) a soccer net was seen in the playground which was deteriorating and in poor condition, no longer making a net, but rather a pile of tangled cords/rope; a base board in the main classroom was seen to be chipped with chipping paint. 5a) several outlets were seen as not tamper resistant
